About the Role

This is an exciting opportunity for a registered nurse to deliver high-quality nursing care to patients in their own homes or community settings. You will work collaboratively within a multidisciplinary team, providing a range of nursing interventions and supporting the holistic needs of patients with acute, chronic, or complex health conditions.

Key Responsibilities
  • Clinical Care: Deliver a range of nursing interventions, including wound care, medication administration, catheter care, and end-of-life care.
  • Holistic Assessments: Conduct assessments of patients' physical, emotional, and social needs to develop and implement individualised care plans.
  • Progress Monitoring: Monitor and evaluate patients' progress, adapting care plans as necessary to achieve the best outcomes.
  • Health Education: Provide health education and advice to patients and carers to support self-management and promote independence.
Requirements
  • Qualifications: Registered Nurse (Adult) with current NMC registration.
  • Clinical Experience: Clinical experience in a relevant setting, such as community, primary care, or acute services.
  • Skills: Strong clinical assessment and decision-making skills, excellent communication, interpersonal, and organisational skills.
