Community Healthcare Specialist

We are seeking an experienced Nurse to join our team as a Band 5 Community Nurse. This is a challenging and rewarding role that will provide you with the opportunity to work in a fast-paced environment, providing high-quality patient care and supporting the delivery of our services.
The successful candidate will be responsible for providing nursing care to patients in their own homes, clinics, and other community settings. You will work closely with a multidisciplinary team to assess patient needs, develop and implement care plans, and evaluate the effectiveness of care provided.
In addition to providing direct patient care, you will also be involved in supervising and mentoring junior staff members, contributing to quality improvement initiatives, and participating in education and training programs.

Key Responsibilities:
- Provide high-quality nursing care to patients in their own homes, clinics, and other community settings
- Assess patient needs, develop and implement care plans, and evaluate the effectiveness of care provided
- Supervise and mentor junior staff members
- Contribute to quality improvement initiatives
- Participate in education and training programs
Requirements:
- NMC Registered Nurse [Adult]
- Previous experience as a Band 5/6 community nurse
- Care planning: being able to assess patients, develop, implement and evaluate a care plan
- HR skills training e.g. recruitment, managing absence
- Empowering and working in partnership
- Knowledge and understanding of Safeguarding/Advocacy
Salary and Benefits:
The salary for this role is £42,939-£50,697 per annum (inclusive). The post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
