Complex Care Support Specialist (Swindon)
We are seeking an experienced Complex Care Support Specialist to join our team in Swindon. As a key member of our team, you will be responsible for providing person-centred and strengths-based support to our customers with complex needs.
Key Responsibilities
- Providing individualised support to customers with complex needs, promoting independence and social inclusion.
- Working closely with the customer's care team to develop and implement support plans.
- Monitoring the customer's mental and physical health, identifying any signs of deterioration and taking positive risk management to their health and wellbeing.
- Supporting the customer to participate in their favourite activities and trying new things through positive risk-taking.
- Collaborating with colleagues to provide a consistent approach to support.
Requirements
- Level 2 Diploma in Adult Health & Social Care or equivalent.
- Experience working in a Complex Care setting, supporting people with complex needs or behaviours of distress.
- Patient and calm approach, able to adapt to customer needs and circumstances.
- Experience of positive risk management, ensuring the safety and wellbeing of customers and colleagues.
- Understanding of Makaton an advantage.
