Role Overview
EPUT, a leading healthcare provider in the East of England, is seeking an experienced Care Team Lead to join their Community Care Coordination Service. As a key member of the team, you will be responsible for coordinating care services for patients living with frailty and long-term conditions.

The ideal candidate will have a strong understanding of the health and social care setting, excellent organizational skills, and the ability to manage time effectively. A UK driving license and access to a car for work purposes are also essential requirements.

This role offers a competitive salary of £25,000 - £30,000 per annum, depending on experience, plus benefits including season ticket loans, NHS discounts, and excellent training facilities.

Key Responsibilities:
  • Coordinating care services for patients living with frailty and long-term conditions
  • Managing telephone and face-to-face enquiries from staff, patients, relatives, and carers
  • Providing support for activities of daily living as required
  • Maintaining accurate documentation and information systems

Requirements:
  • Care Certificate or willingness to work towards
  • Good standard of literacy and numeracy
  • Understanding of the concept of promoting independence
