As a Care Coordinator at South Birmingham Alliance PCN, you will play a vital role in supporting patients and healthcare professionals in our practice.

About the Job

The successful candidate will contribute to tackling inequalities in health and social care, particularly regarding individuals with long-term conditions. They will also demonstrate an ethos of promotion of independence and partnership-working.

This is an exciting opportunity for an individual who is forward-thinking, enthusiastic, self-motivated, and keen to work collaboratively. The ideal candidate will have excellent administrative skills, a good understanding of working in General Practice, and the ability to exercise sound judgement.

Main Duties of the Job
  • Receive and collate information from transfers of care (including hospital admissions and discharges) and present this information to the MDT as required.
  • Support the completion of new referrals by checking criteria and directing referrals to the MDT.
  • Act as a point of contact for relatives of deceased patients, ensuring they are kept informed regarding issuing and forwarding of death certificates.
