We are seeking an experienced Registered Nurse to join our Discharge to Assess team at Livewell Southwest. As a key member of our team, you will be responsible for providing high-quality nursing care to patients in their own homes, ensuring a safe and timely hospital discharge.
Key Responsibilities- Conduct comprehensive assessments of patients' needs and develop individualized care plans.
- Work collaboratively with multidisciplinary teams to ensure seamless care transitions.
- Provide education and support to patients, families, and carers to promote independence and self-management.
- Manage complex care coordination, including medication management and equipment provision.
- Participate in clinical governance and quality improvement initiatives to ensure high standards of care.
- 2+ years of recent experience as a Band 5 Nurse in an acute or community setting.
- Ability to demonstrate transferable skills and adapt to changing environments.
- Relevant clinical experience and competence in medicines management, complex care coordination, and community working.
- Excellent interpersonal and communication skills, with the ability to build and maintain relationships with patients, families, and healthcare professionals.
