Job Summary:

The successful candidate will work in a 34-bed rehab facility, providing occupational therapy services to patients who are not ready to return home. The role will involve working 5 mornings per week, with the afternoons spent on community follow-up, case management, and support.

Key Responsibilities:

  • Provide occupational therapy services to patients in a rehab facility setting
  • Conduct community follow-up and case management
  • Support patients in their homes and communities
  • Collaborate with multidisciplinary teams to ensure comprehensive care

Requirements:

  • Experience working in a rehab facility setting
  • Strong knowledge of occupational therapy principles and practices
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills

Working Hours:

The successful candidate will work Monday to Friday, with core working hours of 8am to 4pm. Adaptations, equipment, and moving and handling experience are essential for this role.
