At Marie Curie UK, we are seeking a skilled and compassionate Hub Coordinator to join our team in the South West region. As a key member of our administration and support team, you will play a vital role in ensuring the smooth operation of our care coordination hub.

Key Responsibilities:

  • Act as the first point of contact for Marie Curie Services across the South West, providing exceptional customer service and support to patients, families, and healthcare professionals.
  • Lead and support the referral process, coordinating and booking care as required, ensuring accurate data entry into operational and clinical systems.
  • Work closely with Senior Coordinators to ensure the efficient running of the coordination hub, collaborating with clinical support and management staff, volunteers, referrers, local CHC teams, patients' families, and carers.
  • Utilize your excellent communication and interpersonal skills to negotiate with Marie Curie Nurses regarding shift coverage, ensuring seamless care delivery.
  • Input new patient details into Marie Curie systems, ensuring accurate and up-to-date information is available to nurses.
  • Communicate with families regarding care provision, liaising with referrers to confirm care packages before offering patients care.
  • Ensure all systems are up-to-date with information for booked visits, enabling Marie Curie Nurses to provide effective care.
  • Coordinate last-minute changes advised by the Clinical Nurse Manager, Hub Manager, and Senior Nurse.
  • Listen to patients' and carers' concerns, suggestions, and experiences, signposting them to relevant service designers and managers.

Requirements:

  • Excellent communication, interpersonal, and organizational skills.
  • Experience working in a customer services or similar environment.
  • Excellent attention to detail and accuracy.
  • Ability to work in a multitasking environment.
  • Proactive approach to problem-solving.
  • Excellent IT skills.
