Job Summary

We are seeking a highly skilled and compassionate Hub Coordinator to join our administration and support team in our Tiverton Office. As a key member of our team, you will be responsible for coordinating and supporting the referral process, ensuring accurate data input, and providing exceptional customer service to patients, families, and healthcare professionals.

Key Responsibilities

  • Act as the first point of contact for Marie Curie Services across the South West, providing timely and accurate information to patients, families, and healthcare professionals.
  • Lead and support the referral process, ensuring seamless coordination and booking of care services.
  • Collaborate with Senior Co-ordinators, clinical support staff, and volunteers to ensure the smooth operation of the co-ordination hub.
  • Communicate effectively with patients, families, and healthcare professionals, providing empathetic and professional support.
  • Monitor and analyze data to identify trends and areas for improvement, contributing to the development of effective care services.
  • Develop and maintain strong relationships with healthcare professionals, referrers, and community partners to ensure seamless care coordination.
  • Participate in continuous professional development, staying up-to-date with best practices and industry developments.

Requirements

  • Excellent communication, interpersonal, and organizational skills.
  • Experience working in a customer-facing or administrative role, preferably in a healthcare or social care setting.
  • Strong attention to detail and accuracy, with the ability to work in a fast-paced environment.
  • Ability to work effectively in a team, with a proactive approach to problem-solving.
  • Excellent IT skills, with proficiency in Microsoft Office and other relevant software.
