Job Title: Social Care Assessors

Cheshire East Council is seeking enthusiastic and self-motivated Social Care Assessors to join our innovative and friendly Adult Social Care Teams. These temporary posts are initially for a period of 12 months and will be reviewed as required.

About the Role

As a Social Care Assessor, you will work closely with our Adult Community Teams to support and assess individuals discharged from hospital or preventing hospital admissions. You will provide a wide range of learning and training opportunities and work collaboratively with the Health and Social Care sector.

Key Responsibilities
  • Conduct thorough assessments of individuals' needs and develop personalized care plans.
  • Work closely with the Reablement service to support individuals in their recovery and prevent hospital admissions.
  • Collaborate with the Adult Community Teams, GP's, Matrons, Mental Health services, Housing, and voluntary sector services to provide comprehensive support.
  • Develop and maintain accurate and comprehensive client documentation.
  • Work autonomously within the Council's policies and professional practice.
Requirements
  • Appropriate Health and Social Care qualification.
  • Experience of adult social care and ability to work under pressure.
  • Excellent communication and organizational skills.
  • Ability to remain calm in a variety of situations.
  • Flexibility to work autonomously and as part of a team.
