You will work as part of an integrated health and social care team, providing crisis intervention and early supported discharge services for patients across the region.
Support the Clinical Lead and Nurse Team Leads in providing clinical knowledge and leadership to the Urgent Response Service, supervising and developing junior staff, and contributing to divisional business delivery objectives.
Collaborate with health and social care partners to establish a whole systems approach to prevent health and social care crises and support early and safe discharge from hospital.
Support the Early Supported Discharge from hospital by assessing patients in their own homes and providing intense therapy as required.
Key Responsibilities:- Manage a designated caseload of clients requiring urgent intervention and organise and prioritise required programmes of treatment.
- Work within the community and in UHS ED as a lone practitioner, managing situations of risk with telephone support.
- Undertake multi-factorial assessments using the Single Assessment Process and undertake specific Physiotherapy assessments for clients with acute, complex, rehabilitation, or palliative care needs.
- Assess Safeguarding risks and refer through the appropriate route, both in and out of hours, using recognised pathways and policies.

Requirements:
Degree or Diploma in Physiotherapy, State Registered. Post Registration Course e.g. rehabilitation, palliative care, chronic disease management, history taking.
