Job Summary

We are seeking a dynamic and motivated Band 6 Caseload Manager to join our Stort Valley and Villages Integrated Community Team.

Main Responsibilities
  • Provide leadership and support to a team of multidisciplinary professionals in delivering clinical health and social care services.
  • Set and maintain high standards of care, deploying, coordinating, and supervising the nursing team as required.
  • Be responsible for the assessment, planning, implementation, and evaluation of individualized nursing care for patients living in the community.
  • Maintain accurate patient and other records.
  • Participate in the long-term management of patients within the locality.
  • Have continued responsibility for a defined caseload.
About the Role

This is an exciting opportunity to work in a community setting, providing high-value healthcare to patients. You will be part of a small integrated community team, working closely with Band 7, Band 6, Band 5, and Band 3 nurses.

Requirements
  • Registered Nurse with a Specialist Nursing Practice (District Nursing) Degree or willingness to undertake training.
  • Nurse Prescribing or willingness to undertake training.
  • Evidence of personal development and experience at Band 5 Community Nurse level.
  • Management experience, including managing a team and managing change within a complex environment.
  • Excellent communication and interpersonal skills.
