About the RoleThis is an exciting opportunity to work in a strengths-based model of practice, supporting residents to improve their independence and wellbeing by working closely with partners and maximizing the use and benefit of community resources.
Key Responsibilities- Undertake strengths-based assessments, reviews, and support planning to enable residents to maximize their independence and improve their quality of life.
- Provide practical support to residents and carers, including accessing living aids and the welfare benefit system.
- Assist in arranging short-term care and providing clients and carers with respite to reduce carer breakdown.
- Work with adult clients, relatives, carers, and communities to help them make informed decisions and contribute to support planning.
- Liaise with colleagues across the Haringey Health and Social Care Partnership to gather information relevant to assessment and support planning activities.
- Maintain and update reliable, timely, and accurate case notes, systems, and other records.
- QSW status, including qualification, Social Work England registration, and completed ASYE.
- A minimum of 2 years' experience.
- Right to work in the UK.
