Discharge Support Professional Job Description:

We are looking for a skilled and compassionate Discharge Support Professional to join our team at the Royal Devon University Healthcare NHS Foundation Trust. As a key member of our discharge team, you will play a crucial role in supporting patients and families during the transition from hospital to home or other community settings.

The successful candidate will require a driving licence and access to a car to complete community visits. A full-time position is available, but we are also open to discussing part-time hours. We offer a supportive environment with regular supervision, CPD groups, training, and peer support. You will also have access to a Champions Network that links you with therapists who are keen to share their knowledge of special interests.

A key aspect of this role is coordinating the smooth running of the service, which requires excellent IT skills. You will utilize our online patient record system, MyCare, to ensure seamless communication and coordination between teams.
