Nurse Care Specialist

We are seeking innovative, caring, and dedicated registered Nurses to join our Redbridge Community Health team. As a member of our multi-disciplinary team, you will work collaboratively with District Nurses, Specialist Nurses, community matrons, therapists, and Health Care Assistants to deliver high-quality, evidence-based care to patients in their own homes.
About the RoleYou will be required to work autonomously, providing packages of care that involve other agencies, and have experience of working as a reflective practitioner. A full UK driving licence and access to a car for work purposes are essential, as you will be travelling around the borough to deliver care to patients.
Key Responsibilities- To provide holistic health care to support the delivery of NELFT's strategic vision of 'Best care by the best people'
- To work collaboratively with Local Authorities, Integrated Care Networks, GP's, and Acute sector providers to facilitate care pathways that prevent hospital admission
- To travel around the borough to deliver community-based care to patients in their own homes
The ideal candidate will possess current NMC registration or any other 1st-level Nursing qualification, and evidence of post-registration education and training. A mentor qualification and relevant post-registration courses in wound management, diabetes, leg ulcer care, continence care, and assessment are desirable.
BenefitsThis post attracts a High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and a maximum payment of £5,735 per annum pro rata). New starters will also undergo a comprehensive induction programme, which includes mandatory training, systems training, and the allocation of equipment.
SalaryThe estimated salary for this role is £38,890 - £44,041 per annum, depending on experience.
