About the Role

We are seeking a dedicated and compassionate Mental Health Practitioner to join our established team at Petersfield's Community Hospital. As a key member of our multidisciplinary team, you will play a vital role in delivering recovery-based community mental health services to adult individuals with severe and enduring mental health needs.

Main Responsibilities
  • Provide recovery-focused assessments, risk formulations, care planning, crisis and contingency planning, evidence-based interventions, response, reviews, reports, discharge planning, and attendance at appropriate meetings.
  • Coordinate and provide recovery-based community mental health services within a multidisciplinary team.
  • Make links with Primary Health Care teams.

Person Specification

Essential qualifications include a Registered Mental Health Nurse, Mental Health Practitioner, Occupational Therapist, or Social Worker qualification. Recognised mentorship training and Nurse Mentorship or equivalent are also required.
