About the Role

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our established team at Petersfield's Community Hospital. As a key member of our multidisciplinary team, you will play a vital role in delivering recovery-based community mental health services to adult individuals with severe and enduring mental health needs.

As a Mental Health Practitioner, you will work collaboratively with individuals, families, and carers to provide assessment, care planning, and evidence-based interventions. You will also make links with Primary Health Care teams to ensure seamless care coordination.

Key Responsibilities
  • Provide recovery-based community mental health services to adult individuals with severe and enduring mental health needs.
