Job Title: Senior Key Worker

We're seeking a compassionate and skilled Senior Key Worker to join our team. As a Senior Key Worker, you'll play a vital role in supporting our clients to live independently, achieve their goals, and maintain their dignity.

Key Responsibilities:

  • Assess and meet the needs of your caseload of service users, including their cognitive and physical needs.
  • Liaise with Multi-Disciplinary Teams to organize the care needed for the service user.
  • Support service users with daily living tasks, such as personal care, medication management, and household chores.
  • Develop and implement personalized care plans to promote independence and well-being.

Requirements:

  • 1 year of social care experience, with a strong understanding of care principles and practices.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and colleagues.
  • Able to work independently and as part of a team, with a flexible approach to meet the needs of the service.
