Care Coordinator

We are seeking a highly skilled and experienced Care Planner to join our team at Wakefield Council. As a Care Planner, you will play a crucial role in ensuring the efficient and effective delivery of our reablement, long-term care, extra care, night care, and day services.
Key Responsibilities:- Manage and coordinate the delivery of care services to service users, ensuring that their needs are met and their expectations are exceeded.
- Develop and maintain strong relationships with service users, their families, and carers, providing them with support and guidance throughout the care planning process.
- Work closely with our reablement outreach practitioners and managers to ensure seamless service delivery and effective communication with service users, staff, and managers.
- Make informed decisions regarding changes to service delivery, taking into account the needs of service users, staff, and the organization as a whole.
- Communicate effectively with service users, staff, and managers, ensuring that all parties are informed and engaged throughout the care planning process.
- English and maths equivalent to GCSE pass level.
- Strong communication and interpersonal skills, with the ability to build and maintain effective relationships with service users, staff, and managers.
- Ability to work independently and as part of a team, with a strong focus on delivering high-quality services to service users.
- Knowledge of issues relating to older people and physical disabilities, with a strong understanding of the reablement and care planning process.
- A full-time working pattern of 37 hours per week.
