Job Description

We are seeking a highly skilled and experienced Advanced Nurse Practitioner to fill the role of Clinical Lead in Community Care. This is an exciting opportunity to work with clients who require specialized support, providing expert clinical practice and guidance.

The successful candidate will be responsible for developing, implementing, and evaluating clinical interventions and strategies, working closely with the Community Learning Disability Team. They will also provide leadership and guidance to colleagues, promoting a culture of collaboration, innovation, and excellence.

Key Responsibilities:

  • Lead clinical assessments, formulations, and interventions, working with service users, families, and other professionals.
  • Develop and implement clinical guidelines, protocols, and procedures, ensuring safe practices and adherence to policies and procedures.
  • Collaborate with the Community Learning Disability Team to design and deliver individualized care plans, promoting recovery and well-being.
  • Conduct audits and evaluations to ensure high-quality services, identifying areas for improvement and implementing changes as needed.
  • Supervise and mentor colleagues, promoting professional development and growth.

Requirements:

  • Postgraduate qualification (masters or equivalent level) in a relevant field.
  • Evidence of post-basic education in Positive Behavioral Support and/or Recovery Model of Care.
  • At least 5 years of experience working with individuals with learning disabilities, mental health issues, and complex needs.
  • Excellent leadership, communication, and problem-solving skills.
  • Audit and research experience relevant to clinical areas.
