We are seeking an experienced Adult Nurse Manager to join our team at Leaders in Care, a progressive provider of complex care packages within the community. This role will be based in the Twickenham area, where we have secured further packages during an exciting growth phase.

About the Role

The successful candidate will be responsible for providing face-to-face support to adult clients and their families, while creating personalised care plans tailored to each individual's needs. You will work closely with a core team of carers, training and supporting them with their clinical skills and activities.

Key Responsibilities
  • Design and deliver a comprehensive care plan that addresses the unique needs of each client
  • Effectively manage multidisciplinary teams through strong communication and collaboration
  • Provide ongoing monitoring, support, and assessment of clients' physical and physiological needs
  • Deliver nursing interventions, including medication management
  • Build and maintain strong relationships with clients' families
  • Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke care packages
Requirements
  • RGN with an active NMC PIN
  • Supportive management style that brings out the best in others
  • Up-to-date experience of tracheostomy care and ventilators within the last 2 years
  • Advanced communication skills for effective liaison with patients, families, and multidisciplinary teams
  • Suitable for Band 5/Band 6 Nurses with evidence of effective leadership and management of HCAs
  • Full UK Driving Licence desirable but not essential
Benefits
  • Estimated salary: up to £45,000 per annum (dependent on experience)
  • Flexible working arrangements, including 1 day remote work per week and 4 days in the community
  • Mondays to Fridays (9-5) hours
  • 33 days annual leave
