Physiotherapist for Complex Rehabilitation

**Job Summary:**
We are seeking a skilled physiotherapist to join our team at Central London Community Healthcare (CLCH). As a Specialist Physiotherapist, you will work closely with our dedicated team to deliver high-quality, patient-centred care to residents of Brent.
**Key Responsibilities:
- Conduct uni-professional and multi-professional assessments, developing and implementing tailored treatment plans that address a range of conditions, including neurological, musculoskeletal, osteoporosis, learning disabilities, and mental health diagnoses.
- Collaborate closely with Brent Adult Social Care and Reablement teams to deliver high-quality, integrated care and promote independence and quality of life for each patient.
- Receive comprehensive induction and ongoing support to help you excel in your role, with access to training, professional development, and opportunities to advance within CLCH.
**Requirements:
- Qualified physiotherapist with clinical experience in community rehab, ideally with a background in neurological, musculoskeletal, or reablement-focused care.
- Passionate individual committed to patient-centred care and collaborative work within an integrated health and social care team.
