Job Summary


We are seeking a skilled Rehabilitation Support Worker to join our established inpatient team at East Coast Community Healthcare CIC. The ideal candidate will have previous experience in health or social care environments and be able to work effectively in a team.



Main Responsibilities



  • Provide hands-on care to patients, including assisting with activities of daily living and managing continence
  • Work alongside nursing and therapy teams to deliver care and support patients in achieving independence
  • Support patients approaching end-of-life and their families, promoting effective communication and holistic symptom management
  • Contribute to the learning environment and participate in clinical supervision
  • Undertake urinalysis, observations, and record-keeping as required

As a Rehabilitation Support Worker, you will play a vital role in promoting recovery and well-being for our patients, working in partnership with nursing and therapy teams to deliver high-quality care.



Person Specification



  • Essential qualifications: evidence of personal development, good levels of numeracy and literacy
  • Desirable qualifications: Level 2/3 NVQ in Health and Social Care or equivalent experience
  • Essential experience: previous relevant experience in health or social care environments, awareness of the needs of older people
  • Desirable experience: previous experience in NHS/Social Care/Voluntary organizations, understanding of Intermediate Care Services
  • Personal attributes: team player, approachable, reliable, flexible, and willing to work all shift patterns
