Key Responsibilities:

  • Provide expert nursing care to patients with complex symptoms and needs
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with the interdisciplinary team to ensure seamless care coordination
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies
  • Provide education and support to patients, families, and caregivers on palliative care and end-of-life issues
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences

Requirements:

  • NMC Registered Nurse
  • Advanced Diploma/Degree in Adult Health Nursing Studies
  • Post-registration courses related to Palliative Care
  • Experience of caring for patients with acute and long-term conditions
  • Demonstrable clinical experience in End of Life Care
  • Previous experience in nursing in the NHS

Desirable Skills:

  • Organisation and planning skills
  • Demonstrable ability with vital signs monitoring equipment and ability to recognise the deteriorating patient
  • Verification of Expected Death training
  • Ability to demonstrate fundamental nursing knowledge and practice skills
  • Knowledge of the End of Life strategy / quality markers/ advance care planning
  • Knowledge of the conditions relevant to End of Life care
  • Awareness of patient experience, quality and safety agendas that support Governance Frameworks
