**Primary Care Network Support Role**

Kcare Health is recruiting a skilled Healthcare Professional to provide support to their Primary Care Network. The successful candidate will have experience working with individuals with severe and enduring mental health.

In this role, you will work closely with mental health professionals, focusing on recovery principles and ensuring people take up annual health checks. You will also refer onto specialist services when necessary and actively support and liaise with other health and social care professionals.

Responsibilities and Requirements:

  • Support team members and leaders in achieving team goals
  • Work independently to ensure individuals receive appropriate care
  • Develop and maintain positive relationships with service users and colleagues
  • Contribute to a supportive team environment
  • Experience working with individuals with severe and enduring mental health
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills

Estimated Salary: £24,000 - £27,000 per annum, based on experience.
