Job Description:

The successful candidate will lead the development and implementation of physical health strategies across Brent Community mental health teams (CMHT), Inpatient services, Learning disability services (LD), Home treatment team (HTT), Older adults (OPHA) and Early intervention services (EIS). They will oversee and undertake physical health checks and health promotion support to complex patients with severe and enduring mental health problems.

The postholder will supervise and support teams to deliver high standards of physical care to patients with long-term conditions in accordance with up-to-date evidence-based professional practice. They will provide leadership to nursing teams within the services, focusing on supporting patients to be healthy, monitoring of long-term conditions, health promotion, and screening.

The postholder will lead on preparation and delivery of health promotion initiatives, linking with Trust resources, local business, and healthcare providers. They will ensure that health promotion materials are up-to-date, available in various languages, and easily accessible across the services.

The role requires the postholder to work collaboratively with multidisciplinary groups to meet the physical health needs of patients within community services.
