**Job Summary**

As a Medical Practitioner - Primary Care Specialist, you will be responsible for ensuring the highest standards of primary care for all registered and temporary patients.

You will assess, examine, diagnose, plan, treat and review/evaluate patients, including those presenting with acute and chronic conditions. You will also identify health priorities and make timely and appropriate interventions to support patients in emergency situations.

The ideal candidate will have NMC registration, QOF knowledge, and excellent communication skills. Experience of SystmOne/EMIS clinical systems and working in a GP practice/Primary Care setting is desirable.

**Key Responsibilities**

  • Achieve high-quality patient outcomes through effective assessment, diagnosis, planning, treatment, and review.
  • Develop and implement management plans, including medication plans and other interventions.
  • Maintain accurate and contemporaneous records of patient encounters.
  • Collaborate with administrators and other healthcare professionals to ensure seamless referrals and continuity of care.
