Job Summary:

We are seeking a skilled and compassionate Community Health Specialist to join our team at Leaders in Care. As a Community Nurse Case Manager, you will play a vital role in supporting clients with complex care needs to live independently at home and within their communities.

Key Responsibilities:

  • Conduct thorough assessments in the community to identify clients' needs and develop personalized care plans.
  • Collaborate with healthcare professionals and other stakeholders to ensure seamless care delivery.
  • Provide expert clinical guidance and support to clients and their families.
  • Manage and coordinate care services to meet clients' evolving needs.

Requirements:

  • Be a registered RGN or RSCN Qualified Nurse with excellent clinical skills in complex care, including peg feeds, ventilators, and tracheostomy management.
  • Have experience in tracheostomy and complex care.
  • Be a car driver, flexible to travel around the North West region.
  • Have experience caring for both adults and children.
