About the Role

We are seeking a dedicated and compassionate Care Coordinator to join our team at Elysium Healthcare. As a Care Coordinator, you will play a vital role in providing high-quality care and support to individuals with Learning Disability and Autism conditions.

Key Responsibilities
  • Conduct ward rounds to assess the needs of service users and develop personalized care plans.
  • Work collaboratively with colleagues to analyze behaviors and adapt care accordingly.
  • Update patient records to ensure accurate and up-to-date information.
  • Provide overall care and support, which may include assisting with personal hygiene.
  • Develop and maintain positive relationships with service users, their families, and other healthcare professionals.
