Key Responsibilities:
- Develop and implement personalized care plans to support individuals in achieving their goals.
- Provide assistance with daily living skills, such as personal care and medication management.
- Facilitate socialization and community access for individuals, promoting independence and self-confidence.
- Communicate effectively with individuals, their families, and external professionals to ensure seamless care delivery.
Requirements:
- Hold a valid UK manual driving licence.
- Excellent communication and interpersonal skills.
- A patient and empathetic approach, with a strong commitment to delivering high-quality care.
- Ability to work flexibly, including evenings, weekends, and sleep-ins.
