Responsibilities Include:

  • Conducting assessments and reviews for vulnerable adults, ensuring their needs are met through tailored care and support plans.
  • Collaborating with various teams, including the Learning Disability Team, Autism Team, Social Care Assessment Team, and First Response Team, to provide comprehensive support.
  • Engaging with service users and their families to facilitate smooth transitions into adult care.
  • Providing short-term support to individuals post-hospital discharge, working closely with reablement services.

Qualifications Required:

  • A recognized qualification in social work (e.g., Diploma in Social Work, CQSW, or Degree in Social Work).
  • Registration as a Social Worker with the appropriate professional body.

Experience and Skills:

  • Proven experience in social care, managing diverse caseloads.
  • In-depth knowledge of relevant legislation, guidance, and local policies affecting social care practice.
  • Strong communication skills, both verbal and written.
  • Familiarity with assessment, support planning, and review processes, employing a person-centered approach.
