Advanced Clinical Practice in Stroke

Job Description: We are seeking a skilled Band 6 Community Specialist Physiotherapist to join our team. As part of the Integrated Stroke Pathway, you will work closely with colleagues in the Community Stroke Team to provide seamless transition from the acute setting to community. Your primary focus will be on delivering high-quality therapy and rehabilitation services to patients with complex needs.
Main Duties:- Deliver advanced clinical practice in stroke rehabilitation
- Work as part of a multidisciplinary team to provide high-quality therapy and rehabilitation services
- Collaborate with other professionals and agencies to ensure effective patient care
- Develop and implement guidelines for stroke rehabilitation
- Participate in audits and research projects to improve stroke care services
- Degree or equivalent pre-registration award in Physiotherapy
- State registration with the Health Care Professions Council
- Recent postgraduate courses relevant to stroke/neurological rehabilitation
- Leadership courses
- ECDL or formal qualifications in word processing and spreadsheets
- Relevant and recent experience working at Band 5 level
- Relevant and recent experience working in stroke/neurology
- Experience of multidisciplinary team working and collaborative working with other professions and agencies
- Experience of guideline development, implementation, and audit
- Knowledge and clinical reasoning skills to support advanced clinical practice in stroke rehabilitation
- Knowledge of the roles of other professionals and principles of partnership working
- Knowledge of clinical standards and ability to implement and audit these in the team
- Ability to work well within a team
- Excellent planning and organisational skills
- Excellent communication and counselling skills including observation, listening, and empathy
- Able to adjust style to suit the situation
