About the Role

We are currently seeking a highly skilled and compassionate Care and Support Coordinator to join our team. As a Care and Support Coordinator, you will play a vital role in supporting individuals at risk of homelessness, those struggling with mental health issues, and those affected by substance misuse. Your primary responsibility will be to ensure the safety and well-being of our service users, as well as your own and the staff around you.

Key Responsibilities

  • Support individuals in a hostel setting and within the community to achieve their goals and improve their quality of life.
  • Work closely with service users to understand their needs and develop personalized support plans.
  • Provide emotional support, guidance, and practical assistance to help service users overcome challenges.
  • Collaborate with other agencies and professionals to ensure seamless support and services for our service users.
  • Contribute to the development and implementation of policies and procedures to promote best practice and continuous improvement.

Requirements

  • Previous experience working with vulnerable individuals in a similar setting.
  • Excellent communication and interpersonal skills.
  • Able to work in a fast-paced environment with competing priorities.
  • Ability to maintain confidentiality and adhere to data protection policies.
