About the Role

We are seeking an experienced Community Caseload Manager to join our multi-disciplinary team at East Cheshire NHS Trust. The successful candidate will have a Specialist Practitioner Qualification in District Nursing and a positive approach to delivering high-quality care to our patients.

Main Responsibilities

  • Provide holistic, high-quality care to patients, carers, and their families in their own homes or community settings.
  • Work closely with the therapy team, ACPs, and GPs to provide rapid and planned expert professional assessment and treatment interventions.
  • Manage individual and team caseloads, assessing care needs, developing, implementing, and evaluating care programs.
  • Provide leadership and support across the care community team, demonstrating emotional intelligence to recognize pressures of self and team.
