The East Cheshire NHS Trust is seeking an experienced Community Caseload Manager to join our multi-disciplinary team. This is an exciting opportunity to drive forward the expansion and modernisation of community nursing services, aligned with the NHS Long Term Plan.
Main Duties
Provide holistic, high-quality care to patients, carers, and their families, working closely with the therapy team, ACPs, and GPs.
Manage individual and team caseloads, assessing care needs, developing, implementing, and evaluating care programmes.
Lead and support the care community team, demonstrating emotional intelligence to recognise pressures of self and team.
