Senior Practitioner

We are seeking a highly motivated and experienced Registered Learning Disability Nurse to join our East and South Cheshire Community Learning Disability Team as a Senior Practitioner. The successful candidate will work as part of a multi-disciplinary team, providing high-quality care and support to adults with learning disabilities and complex health needs.
Main DutiesThe Senior Practitioner will be responsible for managing a defined caseload of adult service users, working autonomously and as part of a team to provide comprehensive care and support. This will include assessing and planning care, delivering training and consultation, and modifying and evaluating practice in line with clinical and service needs.
The successful candidate will have excellent communication and time management skills, with the ability to work across a range of health and social care settings. They will also be able to demonstrate a willingness to be an effective and robust team player, with a clear emphasis on the development and training of others.

Person SpecificationThe successful candidate will have:
- A first-level nurse qualification (RNLD/RNMH/RMN) and evidence of post-registration module training in specialist areas.
- Experience of working with adults with learning disabilities in a community setting, with a focus on complex needs.
- Excellent communication and time management skills, with the ability to work autonomously and as part of a team.
- A willingness to be an effective and robust team player, with a clear emphasis on the development and training of others.
