Senior Practitioner

We are seeking a highly skilled and experienced Senior Practitioner to join our Clinical Team at Cheshire and Wirral Partnership NHS Foundation Trust. As a key member of our team, you will be responsible for leading on/contributing to reviewing referrals to our Children and Young People's Mental Health Hub (CYPMHH), triaging referrals for children and young people, and offering advice on appropriate support and signposting.
The successful candidate will have a strong background in mental health, with experience of working with children and young people, and a proven track record of delivering high-quality care and support. You will be a skilled communicator, able to work effectively with colleagues, families, and young people, and have a strong understanding of the importance of inter-agency working.
This is an exciting opportunity to join a highly motivated team and contribute to the development of our 0-18 Mental Health Access Services in the Community. You will have the opportunity to work with a diverse range of stakeholders, including families, young people, and other professionals, and will be supported by a comprehensive induction programme and ongoing training and development opportunities.
Main Duties of the JobAs a Senior Practitioner, you will be responsible for:
- Leading on/contributing to reviewing referrals to CYPMHH
- Triage referrals for children and young people
- Offering advice on appropriate support and signposting
- Contributing to the development of care and treatment plans
- Working with families and young people to provide support and guidance
You will also be responsible for maintaining accurate records, participating in team meetings, and contributing to the development of policies and procedures.

Person SpecificationThe successful candidate will have:
- A core profession in Social Work, Nursing, or Allied Health Professions
- Professional registration with a recognised national body
- Nationally recognised post-qualification, continuous professional development (CPD) related to the core profession
Desirable criteria include therapeutic training to intermediate/diploma level, experience of working with students/learners, and experience of providing clinical supervision.
