Senior Practitioner

We are seeking a highly motivated and experienced Registered Learning Disability Nurse to join our East and South Cheshire Community Learning Disability Team as a Senior Practitioner. The successful candidate will work as part of a multi-disciplinary team, providing high-quality care and support to adults with learning disabilities and complex health needs.
Main DutiesThe Senior Practitioner will be responsible for managing a defined caseload of adult service users, working autonomously and as part of a team to provide comprehensive care and support. This will include assessing and planning care, evaluating and modifying practice, and leading on quality improvement initiatives.
The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively with clients, carers, and other professionals. They will also have a strong understanding of the issues faced by adults with learning disabilities, including mental health, positive behaviour support, and care programme approach.
Requirements- First-level nurse qualification (RNLD/RNMH/RMN)
- Post-registration module training in specialist areas (e.g. physical health in learning disabilities, epilepsy, dementia)
- Commitment to ongoing learning and development
- Nurse prescribing qualification
