About the Role

The East Cheshire NHS Trust is seeking an experienced Community Caseload Manager to join our multi-disciplinary team. As a key member of our team, you will be responsible for providing holistic, high-quality care to patients, carers, and their families in their own homes or community settings.

Main Responsibilities

  • Provide expert professional assessment and treatment interventions for patients in their own homes or other community settings.
  • Manage individual and team caseloads of patients, carrying continuing responsibility for the assessment of care needs, development, implementation, and evaluation of care programmes.
  • Lead and support the care community team, demonstrating emotional intelligence to recognize pressures of self and team.

Requirements

  • Specialist Practitioner Qualification in District Nursing.
  • Ability to work closely with the therapy team, ACPs, and GPs within the centre.
  • Travel is an essential part of this role; please demonstrate on your application form how you would meet this requirement.
