Job Description:
The Nurse Practitioner in Community Healthcare role at Sanctuary Personnel involves working closely with patients, families, and multidisciplinary teams to provide high-quality care and support. As a skilled nurse practitioner, you will conduct thorough assessments, develop and implement personalized care plans, and collaborate with healthcare professionals to achieve optimal patient outcomes. In this role, you will also have the opportunity to participate in multidisciplinary team meetings and contribute to the development of new programs and services.

Key Responsibilities:
- Conduct comprehensive assessments of patients' physical and mental health needs
- Develop and implement individualized care plans that address specific patient requirements
- Collaborate with healthcare professionals to deliver holistic patient care
- Maintain accurate and detailed records of patient interactions and progress
- Participate in multidisciplinary team meetings to discuss patient cases and share knowledge
