About the Role

We are seeking a highly skilled and dedicated Community Care Specialist to join our team at Cambridgeshire & Peterborough NHS Foundation Trust. As a Band Community Nurse Associate, you will play a vital role in providing high-quality care and support to patients in their own homes.

Key Responsibilities

  • Provide expert care and interventions to patients with clinical health needs in their own homes.
  • Work closely with the wider team to ensure seamless care delivery and patient participation in decision-making.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with other community services and PCNs to provide comprehensive care.
  • Travel independently to patient locations, using a vehicle, to meet service needs.

Requirements

  • Experience in community nursing, palliative care, and care of patients with diabetes.
  • Registered Nursing Associate on the NMC Register.
  • Experience in falls prevention work or willingness to gain skills.
  • Ability to demonstrate a commitment to high-quality service provision.
  • Flexibility to work as part of a team and individually.
  • Good time management and ability to manage individual workload.
  • Enhanced communication and interpersonal skills.
