The Role

We are seeking a Care Support Specialist to join our team of Home Care Assistants. As a Care Support Specialist, you will be involved in providing tailored packages of care that support customers' circumstances and lifestyle preferences. This may include support with domestic home help services, personal care needs, nutrition, hydration, medication, and companionship needs, which may include accessing the community.

Key Responsibilities

  • Provide high-quality care and support to customers in their own homes
  • Develop and implement personalized care plans that meet customers' needs and preferences
  • Work collaboratively with other members of the care team to ensure seamless delivery of care services
  • Communicate effectively with customers, their families, and other stakeholders to ensure that their needs are met
  • Contribute to the development and implementation of care policies and procedures
