About the Role

We are seeking a highly skilled Registered Mental Health Nurse to join our team as a Custodial Care Practitioner at HMP Elmley.

Job Summary

The successful candidate will be responsible for providing high-quality care and support to patients during their initial 14 days in custody. This will involve conducting comprehensive health and social care assessments, developing individualized care plans, and ensuring seamless continuity of care upon release.

Main Responsibilities
  • Perform thorough reception screenings to identify immediate health risks and safety needs.
  • Develop mental health crisis plans for patients at risk of mental health crises.
  • Conduct comprehensive health and social care assessments within seven days of incarceration.
  • Lead on identifying patients whose needs have changed post-court or hospital visits.
  • Collate healthcare information from families, community providers, or prior establishments to improve continuity of care and safety.
Requirements
  • Registered Mental Health Nurse with a degree supplemented by diploma or specialist training.
  • At least two years' experience as an RMN, with extensive clinical experience and supervisory experience of junior staff.
  • Able to establish rapport with difficult-to-engage service users and build effective working relationships.
