Practitioner in Early Custody

We are seeking a skilled Registered Mental Health Nurse to join our Early Days in Custody Team at HMP Elmley. As a key member of our multidisciplinary team, you will provide holistic care to patients in their first 14 days of custody, ensuring their health and wellbeing needs are met.
Main Responsibilities
- Conduct comprehensive health and social care assessments for new prisoners, identifying immediate health risks and safety needs.
- Develop individualized care plans that address patients' health and social care needs, working in partnership with them to ensure their safety and wellbeing.
- Collaborate with other specialist services to ensure continuity of care and through-the-gate arrangements.
- Provide brief interventions for substance misuse and mental wellbeing, including sleep hygiene advice and breathing techniques for anxiety management.

Person Specification
- Registered Mental Health Nurse with a degree supplemented by diploma or specialist training.
- At least 2 years post-qualification experience as an RMN, with extensive clinical experience and supervisory experience of junior staff and students.
- Ability to establish rapport with difficult-to-engage service users, build effective working relationships, and work autonomously.
