About the Role
We are seeking a highly skilled and motivated Associate Practitioner to join our Community Pulmonary Rehabilitation team at Sheppey Community Hospital.
As a key member of our team, you will be responsible for delivering high-quality patient care and contributing to the development and evaluation of our pulmonary rehab pathway.
Main Responsibilities
- Ensure accurate and confidential handling of patient information in line with Caldicott and Information Governance Policies.
- Participate in the ongoing development and evaluation of the pulmonary rehab pathway.
- Undertake clinical observations, including blood pressure, respiratory rate, temperature, heart rate, and SATS.
- Undertake blood tests as requested from registered clinicians/GPs/Consultants, following training in accordance with organisational policy.
- Communicate effectively with the multidisciplinary team to ensure seamless patient care.
- Deliver prescribed pulmonary rehabilitation programs independently.
- Provide accurate and up-to-date information regarding patient progress to qualified practitioners.
- Evaluate patient progress and provide consistent feedback to qualified practitioners.
- Deal with sensitive and emotive situations with professionalism and empathy.

- Experience in a rehabilitation or therapy assistant role.
- Strong passion for leading group exercise sessions.
Desirable
- Relevant qualifications or training in pulmonary rehabilitation.
