About the Role

We are seeking a highly motivated and experienced Clinical Practitioner to join our team in the Dynamic Support Pathway. As a Clinical Practitioner, you will be responsible for managing a caseload of individuals with complex needs, providing high-quality care and support to ensure the best possible outcomes.

Key Responsibilities

  • Work autonomously, taking responsibility for care, quality, and clinical activity for a designated caseload of service users.
  • Contribute to care quality performance and clinical activity by providing professional leadership and guidance, clinical advice, and management in relation to the key dimensions of healthcare quality.
  • Responsible for the assessment, planning, implementation, and evaluation of programs of evidenced-based care to a group of service users according to changing healthcare needs and varying levels of complexity.
  • Work within a multi-disciplinary team to ensure effective care plans and treatments are in place for complex service users.
  • Recognize, prioritize, and respond appropriately to urgent and emergency situations.
  • Work collaboratively and ensure maintenance of effective partnership communication and with other professionals, agencies, and stakeholders to ensure needs are met in relation to care input, support for ongoing care needs, and safe discharge.
  • Ensure that fully accurate, complete, and contemporaneous clinical records are written and maintained for each service user in accordance with agreed policy.
  • Actively participate in the development and implementation and delivery of new ways of working, implementing research-based practice in order to promote ongoing care improvement.
