Job Summary
We are seeking an experienced Senior Social Care Specialist to join our community-based team in Derbyshire. The ideal candidate will have the ability to work autonomously and accountably, with a focus on building positive professional links across the area.

Key Responsibilities:
- To carry out face-to-face interventions in a flexible manner.
- To identify signposting opportunities to support carers and maximize independence for individual wellbeing.
- To lead on safeguarding inquiries and investigations, chair multi-agency meetings, and complete Mental Capacity Act assessments.
- To respond to changing situations, including Duty cover, and triage incoming work effectively.

Requirements:
- Experience of working in a social care setting.
- Ability to travel and build positive professional links.
- Knowledge of safeguarding procedures and Mental Capacity Act.
- Strong communication and problem-solving skills.
