Clinical Specialist Therapist

We are seeking a highly skilled Clinical Specialist Therapist to join our team at East Cheshire NHS Trust. As a key member of our integrated health and social care team, you will play a vital role in the development of our new discharge to assess model.
Your primary responsibility will be to coordinate and facilitate the safe and effective discharge of patients from Macclesfield District General Hospital. You will also provide initial assessments and interventions in the home environment where indicated.
Main DutiesAs a Clinical Specialist Therapist, you will work closely with our multidisciplinary team to promote formal and informal channels of communication. You will be responsible for delivering clinical therapy assessments and interventions for patients across the acute and community settings.
You will also support the delivery of a revised Discharge to Assess model across Cheshire place. This will involve working in an integrated way with multiple teams to deliver high-quality multi-professional care.
Person Specification- HCPC registration
- Clinical educators training
- Postgraduate qualifications in a relevant field
- Membership of a relevant professional body
- Advanced communication and listening skills
- Ability to lead junior colleagues
- Proactive and autonomous working style
- Strong problem-solving and decision-making skills
- Ability to work under pressure and prioritize workload
