Job Summary

The Community Health Care Support Worker will play a crucial role in providing clinical and clerical support to the District Nursing Team. This includes working independently and as part of a team to deliver high-quality care to patients in their homes, clinic settings, and other locations.

The successful candidate will have a strong understanding of healthcare principles and practices, with experience in providing care and undertaking clinical observations. A Level 3 NVQ/QSF in a healthcare-associated certificate or equivalent is essential for this role.

We are looking for an individual who is able to work efficiently and effectively, managing multiple tasks and priorities while maintaining accurate records and stock levels. Excellent communication and interpersonal skills are also required, with the ability to build strong relationships with colleagues, patients, and other stakeholders.
