Job Description

This is an exciting opportunity to join our Acute Clinical Team as an Advanced Clinical Practitioner/Trainee Clinical Practitioner. The successful candidate will be responsible for providing specialist care to patients and contributing to the development of intermediate care services.

Main Duties
  • Providing advanced clinical practice to patients within the acute clinical team
  • Leading and managing the multidisciplinary team to achieve service objectives
  • Developing and implementing strategies to improve patient outcomes and reduce hospital admissions
Skills and Qualifications:
  • Post-registration clinical experience in a speciality area
  • Specialist knowledge of patient pathways and advanced theoretical knowledge
  • Independent prescribing or working towards
